چکیده

INTRODUCTION Focal ossification of the ligamentum flavum (OLF) is a rare cause of thoracic myelopathy. The lower thoracic spine is most frequently affected and the patients present initially with posterior column disturbances followed by progressively increasing spastic paraparesis. The pathogenesis of OLF has not been conclusively established. MATERIALS AND METHODS Five patients with thoracic myelopathy due to OLF underwent decompressive laminectomy and excision of the ligamentum flavum. Magnetic resonance (MR) images consistently revealed a linear or beak-like excrescence, uniformly hypodense on T1- and T2-weighted images, situated posterior to the thecal sac. A comparison of the preoperative neurological status and at followup was done using the Japanese Orthopaedic Association and Nurick scores modified for thoracic myelopathy. RESULTS Decompressive laminectomy with excision of the OLF resulted in significant improvement in motor weakness and gait in 4 (1 excellent, 2 good, and 1 fair) patients who had slow-onset, but progressive compression, OLF. The patient who had acute traumatic spinal injury did not recover despite decompression and rehabilitation. All improved in their gait and spasticity, but 4 patients had persistent sensory deficit. CONCLUSION OLF can significantly contribute to a spatial reduction of the thoracic spinal canal, resulting in slowly progressive paraparesis or acute paraplegia after trauma to the back. The T2-weighted sagittal image of MR imaging is the modality of choice for screening the longitudinal extent of OLF, with increased diagnostic accuracy when combined with computed tomographic myelogram. Neurological improvement usually occurs following decompression laminectomy with or without excision of the OLF. However, the persistence of residual numbness or weakness at follow-up may be due to irreversible changes within the cord as a result of severe thecal compression and the delay between the onset of initial symptoms/signs and surgical decompression. Prognosis remains poor for acute myelopathy with pre-existing OLF, despite surgery.

Chest Discomfort Secondary to Ossification of the Ligamentum Flavum of the Thoracic Spine: A Case Report

Symptomatic thoracic ossification of the ligamentum flavum is rare, and a modality for its treatment has not yet been fully established; most reported patients complained of myelopathy. Herein, the authors will report on an extremely rare presentation of thoracic ossification of the ligamentum flavum, along with clinical outcomes. The patient complained of chest discomfort and myelopathy sympto...

Thoracic myelopathy due to isolated ossification of the ligamentum flavum.

We report a 72-year-old patient with thoracic myelopathy due to isolated ossification of the ligamentum flavum at T9-T10. Severe paraparesis had developed before the lesion was identified when thinning of a segment of the lower thoracic spinal cord was suspected on a second MRI examination. The diagnosis was then established by CT.

Cervical myelopathy caused by ligamentum flavum ossification Mielopatia cervical por ossificação do ligamento amarelo

Ossification of the ligamentum flavum (OLF) is a rare manifestation and when causing myelopathy it affects essentially the thoracic spine1. Only a few cases of symptomatic cervical disease have been reported, with all patients being Japanese2. The first OLF case reported in a Caucasian was performed by Parekh et al., in 19933. Since then, 19 cases have been reported in Caucasian patients and al...

A Rare Case of Ossification of Ligamentum Flavum presenting as Dorsal Myelopathy in Achondroplasia

Ossification of the ligamentum flavum (OLF) was discovered by Polgar and further described by Yamaguchi and Isuruni [1]. Among the thoracic myelopathies, its incidence is less than that of ossification of the posterior longitudinal ligament (OPLL) and higher than that of herniated nucleus pulposus (HNP) [1]. It is mostly prevalent in Japan and other East Asian countries such as Korea and China ...

Progressive Relapse of Ligamentum Flavum Ossification Following Decompressive Surgery

Thoracic ossification of the ligamentum flavum (T-OLF) is a relatively rare spinal disorder that generally requires surgical intervention, due to its progressive nature and the poor response to conservative therapy. The prevalence of OLF has been reported at 3.8%-26%, which is similar to that of cervical ossification of the posterior longitudinal ligament (OPLL). The progression of OPLL after c...

Myelopathy due to ossification or calcification of the ligamentum flavum: radiologic and histologic evaluations.

The clinical, radiologic, and histologic features of ossification and calcification of the ligamentum flavum were studied in 18 patients. Ossification (15 patients) usually occurred in the lower thoracic spine in men of various ages, while calcification (three patients) was found exclusively in the cervical region of older women. Histologic examinations of ossification showed mature lamellar bo...
